OSHA 300 and 300A Forms.
Fill in these forms and once you've submitted your online application scan and send to
stepapps@abc.org as specified in STEP Two below.
Please make sure you redact/delete all names from the 300 form before emailing them to us.
Please put
STEP APP 300/300A and your company name as the title of the email.
Step TwoGet all of your STEP related information in front of you before you open the STEP online application form. This includes your 300 and 300A forms, your Key Componant scores and, if applying for Platinum, a credit card. This will make filling out the online form a much simpler and quicker process.

If you would like to apply for Platinum and pay by check, please download and print the application here and post with copies of your OSHA 300 (with names removed) & 300A forms, to your local chapter with payment.
